MULTI PURPOSE HALL (MPH) OF THE CHAMPIONS MASJID — RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT

In consideration for your being permitted to enter, observe, use or participate in the premises, facilities, equipment, and affiliated programs of the MPH of the Champions Masjid, you agree to the following:

1. You warrant that you have inspected the MPH facilities, and you accept the MPH facilities as being safe and reasonably suited for your entry, observation, use, or participation.

2. You hereby release, waive, and covenant not to sue, and shall indemnify and hold harmless the MPH of the Champions Masjid, its affiliates, directors, officers, employees, volunteers, and agents from and against any losses, liabilities, damages, suits, actions, claims, relating in any way to bodily injury, death, or property damage sustained by you while present in or participating in the MPH facilities, whether caused in whole or in part by the negligence of the MPH.

3. You voluntarily assume full responsibility for any risk of bodily injury, death, or property damage while you or your minor child are present in or participating in the MPH facilities.

4. You assume full responsibility for any items lost or stolen while present in the MPH facilities.

5. You give permission to the MPH to use photographs, film footage, or tape recordings which may include your image or voice for purposes of promoting any MPH facilities.

6. You agree to abide by all rules promulgated by the MPH while present in or participating in any MPH facilities.

7. You understand that MPH membership and program fees are not deductible as charitable tax contributions.

8. This agreement is intended to be as broad and inclusive as permitted by Texas law.

MPH RENTING PROCEDURES AND PROTOCOLS

Scheduling:
• All reservations must be made AT LEAST 48 hours BEFORE the day of the event.

Chairs and Tables:
• Chairs and tables available for ~350 people in room 102 (next to the hall).
• You are responsible for setting up and returning chairs/tables after the event.
• The steel door between main Masjid and MPH closes at 4:00 PM — have chairs set up before then.

Decoration:
• No posters, pictures, or frames may be hung on hall walls.

Catering Food:
• Outside catering allowed, but no kitchen amenities (chafing dishes, burners, etc.) available.

Other Amenities:
• Microphone available upon request — must be returned after event.
• No stage available.

Cleanliness:
• Hall must be cleaned after the event.
• Bring your own cleaning supplies (trash bags, broom, dustpan, paper towels, mop).
• All trash must be bagged and dumped in the outside dumpster.

Hall Rent:
• Rental fee: $700
• Security deposit: $300 (refunded after inspection)

Discounts (cannot be combined):
• 10% — Active ISGH members (personal events only, direct family only)
• 20% — Active Masjid volunteers (must be ISGH member, approved by committee chair and Masjid Administration, personal events only, direct family only)
